Brooks County Middle School issued the following announcement on Jul.19.

Dear Parents and/or Guardians of Upcoming 7th Graders:

Georgia Law requires 7th graders to have one dose of the Tdap Vaccine and one dose of the Meningococcal Conjugate Vaccine for individuals who have not already received those vaccines.

Please be prepared on the first day of school to provide proof of immunization (i.e., Certificate of Immunization {Form 3231} to the front office. If we do not have the actual immunization report, we do not know vaccines were given and your child will be unable to attend school until the immunization form has been received.

Your health care provider or local health department can provide the vaccines and/or issue the Certificate of Immunization Form. For additional information about the vaccines and diseases they protect against, visit the Center of Disease Control and Preventions' website at www.cdc.gov/vaccines/who/teens/index.html.
